请教填写 training employment pass application form 的几个

我是中国要去新加坡的访问学生，对新加坡的大学开展几个月的合作研究，对方要求我填一个training employment pass application form，可能用来申请签证之类的吧，有几个问题想请教一下：
1: 2A： Is the applicant here for practical training attachment? 我这种情况是yes还是no呢？
2: 4B: Societies/Organisations Membership 部分，由于我一直在上学，这一部分填什么呢，还是空着呢？
3：5A applicant's employment details, 问题同上，我一直在上学，这一部分是空着吗？
4：5B： fixed monthly salary 些什么呢？ 5C：Trainee‘s occupation 写什么呢？
