New versions of application forms and changes to a

The UK Border Agency has published new versions of application forms for many temporary and permanent migration routes.
All FLR and SET applications forms, and forms for applications made in the UK under the points-based system, have been revised to reflect:

the mid-year increase in application fees;

the new option to pay the application fee by American Express (Amex); and

additional declaration pages which will allow us to obtain the information needed to support applications made in the UK.Version 11/2010 of these forms should be used for all applications made from today.
The guidance notes for FLR and SET forms have also been revised (version 11/2010).
